Please download our Application Form, Terms & Conditions, the Lambeth Questionnaire and the Allergen Matrix below. Forward, along with your food trading documents, your Public & Employers Liability; local authority registration/food hygiene rating (levels 4 or 5); food safety certificates; Food Safety, H&S and Fire Risk Assessments. For traders using Power or Gas please include a full PAT and/or Gas certificate.

All applications must include a full product list/menu, price list including where the produce is sourced from. Allergen lists must be included for all pre-packed foods. Without these documents, applications will not be considered.
